Refund Administration Fee

There will be a $25.00 non-refundable administration fee deducted from any registration fee if the player decides not to play.

Online 50/50

Port Lambton Athletic Association is hosting an online 50/50 for Gala Days 2024 (this will be in place of the 50/50 tickets that players have received in the past with registration).

Mandatory Volunteer Hours

Every family registered with PLAA will be required to submit a $100.00 volunteer cheque post dated for September 1, 2025. Cheques are to be made payable to Port Lambton Athletic Association. You will receive your cheque back once you complete 2-4 volunteer hours. 

If you did not complete your hours in the 2024 season and did not submit a cheque, you will be required to pay the $100.00 volunteer fee prior to registering for 2025.


* If you cannot provide a cheque and you do not complete you 2-4 hours, the $100.00 fee will be added to your 2026 registration costs. 

To offer these community programs at such a minimal cost, it is imperative to have your support and help. All parents/guardians will be required to complete 2-4 hours of volunteer service per family in the below noted areas. Upon completion of these hours & your uniform returned your cheque will be returned, if the hours are not completed your cheque will be cashed.

The volunteer opportunities listed below, are the only ones that hours will be recognized:

- Head Coach / Assistant Coach / Team Manager
- Tournament Convenor / Helper
- Working in the concession booth
- Gala Days (helper / setup / takedown)
- Park and Diamond Maintenance
- PLAA Executive/board Member

* Game changer, score keeping and pitch counts are considered team responsibilities. These are not recognized volunteer hours. 
Dual Registration:

By completing this form, you are registering with both Port Lambton Athletic Association (PLAA) and Baseball Ontario. 

Baseball Ontario appoints PLAA to collect fees on its behalf, included in the registration fee collected through this process. No additional monies is charged to the parent registering locally. Baseball Ontario invoices PLAA for fees collected.
Liability Waiver:

On behalf of myself and the player being registered, I recognize that baseball entails serious risks. Consequently, except as noted below, I and the player being registered relinquish all rights to a claim of any kind, including the right to a claim for bodily and material damages, regardless of the cause, against Baseball Ontario, its member associations, including Port Lambton Athletic Association (PLAA) and their respective officers, employees, coaches, umpires, assignees, agents, representatives, and sponsors, even if such damages result from negligence of Baseball Ontario, its member associations, including PLAA and their respective officers, employees, coaches, umpires, assignees, agents, representatives, and sponsors.

Without restricting the generality of the preceding, I and the player being registered also relinquish the right to any claim against Baseball Ontario, its member associations, including PLAA and their respective officers, employees, coaches, umpires, assignees, agents, representatives, and sponsors resulting from a decision on their part, regardless of the nature of this decision.

Photo Consent:

I authorize Port Lambton Athletic Association (PLAA)  and/or its staff, associates, assistants, or subcontractors to use photograph(s), video and other media image provided for any PLAA promotional, educational or other pertinent uses. I also agree to PLAA using any images of the named minor(s) gathered by photographers, and or videographers supplying images to PLAA .

These images may include, but are not exclusive to, club, coach, athlete and/or parent, and/or volunteer submissions by various member associations of PLAA

I authorize PLAA  to permit the use and display of photographs and/or recordings of the named minors in any PLAA  publication, multimedia production, including video and web usage, display, or advertisement.

I agree that PLAA may use name, likeness, or information supplied by the undersigned.

The undersigned releases and forever discharges PLAA and its officers and employees from any and all claims and demands arising out of or in connection with the use of said photographs / recordings, including but not limited to, any claims for invasion of privacy or defamation.
 

Rowan’s Law:

Under Rowan's Law, before any player can be registered with the local association and Baseball Ontario, the player, and the parent or legal guardian of the player if the player is under 18 years of age, must review one of the Concussion Education Resources provided by the Province of Ontario and must review the Player Code of Conduct. Rowan's Law Acknowledgement:

I hereby confirm that the player being registered to participate in baseball activities with the local association and Baseball Ontario, and the parent or legal guardian of the player being registered if the player is under 18 years of age, have reviewed one of the concussion education resources provided by the Province of Ontario as referenced above and have reviewed the player code of conduct as referenced above. I / we further acknowledge understanding the nature and risk of concussion and head injury to athletes, including the risks of continuing to play after a concussion or head injury is suspected.

Registration Payment Information

Payment will be done using E-Transfers for the 2024 season. We will not be accepting any other from of payment.

Immediately after filling out your online registration please make payment using the email address [email protected] 

Please include your player's first and last name in the comment section of the E-transfer. If you are paying for multiple registrations please include all names.


You will NOT receive registration confirmation until both steps are complete. You must be paid in full by November 30, 2024 or a $75.00 late fee will apply.
